Apply to create a COS Fund
What is a C.O.S. Fund? A Fund that all monetary donations are tax deductible and managed for an individual who is going through a medical crisis. Whether you are a business wanting to contribute funds for your employee; a relative, friend or neighbor who would like to donate money to your loved one who is going through a medical crisis; or an individual who wants to seek funding and contributions, the COS Fund can be established to assist with those needs.
Benefits
- Assistance with the solicitation letters sent out to the co-workers, business associates, families, friends, neighbors, etc.
- Promote the COS Fund by having your own webpage located on CBT website to display all COS fund information.
- Access to have Paypal on your COS Fund website page
- All donations will be tax deductible for those who donate to your COS Fund.
- Manage the COS Fund account by recording all donations to your fund, sending out tax deductible reciept, thank you's and the funds to pay the bills for the recipient.
- Unlimited advice or answering of questions about the COS fund.
- The ability to change the COS fund from a fund status to a Celebration of Support event status. Administrative Fee of $500.00 would apply and a COS Event Agreement form would have to be completed and mailed to Community Benefit Tree, Inc.
- You can add donations from brat frys, bake sales, dress down days or other small fundraisers.

The person or business that is initiating the fund will have to complete the agreement form click here. Make sure everything is filled out completely.

Set Up Appointment: If you would like to set up an appointment to get more information or to answer any questions you might have before you fill out the application or agreement just call or email Heidi to setup that appointment.We will be happy to meet with you, the recipient and the committee.
By filling in the following application and agreement you are ensuring us that your recipient is someone who is real and has a real need and the agreement lets us know that the committee wants our help and is willing to follow the rules and regulations of the Community Benefit Tree. After filling out the forms, you can email them to heidifrederickson@communitybenefittree.org , fax it to (920) 766-4387 or mail it to Community Benefit Tree, W3494 Dundas Rd, Kaukauna,WI 54110 making sure you have ALL the forms and letters submitted together.
Process: After having received the agreement,application and the two referral letters, the Board will look it over for approval. This will take 5 to 7 days. Once approved, we will be in contact with you to start assisting with your benefit. Any questions can be directed to Heidi at the above email or phone number.
